Checkbox

A simple checkbox for recording yes/no answers. Note that an un-ticked checkbox can either mean "no", or "not answered". If you need to tell the difference, use a dropdown list with the values "Yes" and "No".

If this input type is mandatory, the user must tick the checkbox.

This type of input does not require parameters.

You can type the word 'yes' (without the quotes) into the Default Values box to have the checkbox ticked by default.

This form item type does not support allowed value images.

This form item type does not support Query String population.
